What is Selform?
Tamisemi Selform is an online platform designed to streamline the selection process for secondary school placements (Form Five) and higher education opportunities (colleges) in Tanzania.
SELFORM MIS portal uses candidate registration data from the National Examinations Council of Tanzania (NECTA). This data has been packaged together with the application, and it is already available in the system after installation.

How to Change Combination for Form Five Students (Selform MIS – Tamisemi)

Steps
Step 1: Open your browser (example, chrome), type the following address selform.tamisemi.go.tz to get a window to fill in your registration information.
Click on the bottom menu labeled For Candidates, Click here to Register if you are a first time user.
Step 2: Then, fill in the relevant information, for Index Number follow this format Example S0101.0020.2024, Answer the question you will be asked, Surname and Year of Birth as shown below.
Step 3: Then a window will open asking you to enter a Password that you will use every day. The following window will open;
Step 4: Once you have entered your password, the system will appear in this form to show that you have successfully changed your password.
